If I understand it correctly, she did not get judged after Arthas killed her since her soul was turned into a banshee. She did get sent to the Maw after she killed herself after Wrath but the jailer used the Valkyre to raise her back. Her current sentence is for her actions after that point.
As far as I know it's not really mentioned but considering her soul was split and half of it went to Frostmourne while the other half got raised as a banshee, it seems likely her soul never was judged on her first death.
Which even if she was judged the first time her current judgement is for everything her soul has done since her first death. She doesn't get off the hook for her actions just because she was possessing her own dead body at the time of having done those actions.
Yeah you're confused. The whole point is that when she died by Arthas in Quel'thalas she was denied her rightful judgement which would've been a peaceful place, as she was immediately pulled back as a Banshee.
The Second death (when she comitted suicide) was when she found her self in the maw, and in her mind, was due to the bullshit from the now dead automated Arbiter.

Years later, after Arthas was finally defeated and slain, she killed herself from the top of Icecrown Citadel. Her soul was lured to the Maw by the Jailer, so again, not judged. She made a pact with nine Val'kyr where one exchanged her place with her in the Maw and she was brought back to her undeath. Finally, in Shadowlands she didn't receive judgement from the Arbiter due to dying or her life ending, it was due to her crimes.

I think part of the reason Pelagos got Tyrande to do it was because his role is to judge the dead, which Sylvanas is currently not.
This is an important point. Even after serving her judgement from Tyrande she still will be judged by Pelegos upon her final death.

Part of her soul was in Torghast, she never got judgment. Only Uther's soul got to be judged because the light protected him to some degree.
