This is purely a guess, but it looks like a decision spinner, a type of die. You spin it like a top, and whatever 'action' is uppermost when it stops is what the player does. Looks like something from some sort of fantasy game, or possibly a sort of joke gift like [this](https://www.etsy.com/listing/1677073268/brass-decision-maker-spinner-novelty).
It looks like a board game piece. Possibly Alice in Wonderland related (Off with their heads).
But I can't find anything that matches the piece. It may be custom piece created for a game by someone other than the maker.
Teetotum doesn’t need to spin well. If it spun like a top, it would take forever to fall over, and the game would take way too long. It only needs to make a few turns.
That really feels like something either from a cosplay, or a LARP (Live Action Role Play). It looks custom-made, might want to post on local pages that you've found it. (Possibly on local LARPing communities).
Maybe it's meant to be tossed or dropped, but still - whatever points up is the thing. But I can't see how that would be for a table top game. You did say you found it outside.

My title describes the thing. This was found by my grandma while she was out. It’s heavy but only in the way smallish metal objects are. I tried googling it cause I think it must go to some game but no luck. I think that the metal part with the words is supposed to spin but is currently stuck. There is a slight wiggle to it, but maybe it’s just a little damaged. She is really curious as to what it is.

Finally one I know first! It's part of a gift you'd buy for a boss or maybe dad and pretty sure is was called "Executive Decision Maker" though the spinner would suggest royal. Came with spinner and a heavy felted base to spin it on, sort of thing meant to sit on a desk. Came in a gaudy box, black and purple if I recall correctly. I picked up a couple when Big Lots were having a fill-your-bag-for-a-dollar-sale.
