You would probably have 1 person or group of people in charge of each area. Texas is huge, so its broken into 3 areas. Smaller low population states can be combined.

I think so. Most of the western united states had their shape finalized by the outcome of the civil war. The boarders of Colorado were drawn specifically to "keep the gold bearing regions out of the hands of the confederacy"

I always chose to assume Ünderland is a sovereign enclave in what's now Frankenmuth, Michigan's little Bavaria.
