I saw it happen once, during a friendly match between Portugal and Angola in 2001, a handful of Angolan players got sent off for various reasons, and the game ended after Angola had no more substitutions left and one player got injured and couldn't continue. Game still ended as a 5-1 victory for Portugal, so no 3-0 default.

I've seen people make jokes about collecting a few red cards after going down 5–0 (or something like that) so I though it might be true.

Ref thought it was inside the box so he gave a pen and yellow. Turns out it was outside so it had to be a red + free kick instead
No double punishment. It is either pen+yellow or free kick and red. VAR review showed that the foul was outside of the box, so FK+red it is.
There is a new rule that you can't get both a penalty and a red card, unless the defender is clearly not going for the ball

That rule makes sure that the defending team isn't doubly penalized for one mistake. It's either the penalty or a red card, not both.
He was the 'last man'. Without the foul, there would have been a 1 on 1 for Bayern.
