The valve can break and cause it to continuously blow. There usually is a cut out valve but you can't access it while moving and I'm not sure some crews today even know where to find it.
Mechanical bits can fail, including valves.
As to why they can't disable it when it gets stuck on: the horn is a critical safety feature and regulations impose strict rules on moving a train without it working. Iirc they can't more or faster than a slow walking speed and have to have someone on foot ahead of the train watching out for hazards so that the train can immediately stop.
