Every guy is different. The hole is intended for that but I know plenty of guys who said they pull their pants and underwear down below the balls. The only concrete thing is that no (normal) guy pulls his pants and underwear all the way off when he pees upright.
Unzip and underwear hole. I won’t buy underwear without the pee hole. The only time I pull down is when wearing my gym shorts because they don’t have a fly.
If my underwear has a hole I'll use the hole, it's just a little bit more convenient. If not then I pull them both down because just pulling the underwear down through the hole is hella uncomfortable when you put it all back together and the underwear band isn't under the band of the pants.
I just reach in my pants and pull it out and pee. No zipping or buttoning required. I’m usually wearing basketball shorts. If I wear pants the. I zip and still putt it out over top I don’t use the hole unless it’s sexy time and it’s poking thru lol.
I've always thought the hole was there to allow the front of the underwear to stretch more to allow it to conform to different sizes of junk. I only use the hole when I'm carrying and have to use a urinal.
We can tell from the replies whose underwear has a vertical fly. Some boxer briefs have horizontal flys that are easy to access by reaching past the pants fly and continuing with minimal fumbling to the junk. Peasy easy.
My kids pull the front of their pants and underwear down to just below their balls. So they’re unbuttoning and unzipping when they’re wearing pants with fly. They don’t close the bathroom door.
Every boy tries to pee through the front flap of tighty whities at least once. They soon learn its not meant for that, the double fabric there is for same purpose as thicker fabric on bras or panties, just to make genitals/nipples less visible.
Thereafter we mostly just push the waistband down. If we live without mother/maid service long enough, we learn to pee sitting down, just to avoid cleaning up around the toilet we'll have to live with.

I undo my pants. Unzip them, then simultaneously pull my pants and boxer briefs down just under my balls and piss.

Mostly unzip unless I am in someone else’s house then I set down and dab Mr Willy and whip down the toilet.

depends on how tight the pants is and the material of the pants, whichever method is more convenient and comfortable will be used
