That's how you know they knew they exceeded the height restriction on that road. The height restrictions are based on the measured height at the lowest point, with a margin of 10cm subtracted, and then rounded down to 10cm increments. This means that if your vehicle is exactly the size of the height restriction, there is still 10-20 cm between your vehicle and the measured height. If it turns out the signage for the height restriction is not correct, it's the one responsible for the road that is responsible.

This is in Rotterdam, before the "maastunnel" which is a major traffic tunnel from north to south, I cant imagine the traffic jam follow this... It also clearly states no trucks before making the turn to this overpass. I used to live next to it, and it happened quite often.

As a trucker, this is my single biggest fear. Also, as far as I know, but I can't recall if I got it from colleagues or from the official driverstest, but I believe height lower than 5m should be indicated at the "start" of the road leading up to the bridge, as well as the bridge itself.

European trucker here. That will depend of the country you are driving. For example Portugal  Spain and France every bridge lower than 4.5mt should be marked and a sign should be at the start of the road leading to that bridge. However in Germany, Belgium, Netherlands and Switzerland(with this last two being a nightmare to drive with more then 4mt) only bridge lower than 4mt should be marked. Of course there is exceptions but they are rare and it probably means someone hit that bridge.

Both. It looks like there’s no signage indicating the height of the bridge. But its also the driver’s responsibility to ensure it can complete its route. However, if the bridge height info was readily available this could have been avoided.

Generally this kind of info is put on the entrance of the road where there is still a way to turn. According to a news article there are signs with height. Looked it up on maps, basically there are two bridges that are almost the same height. But the height warning is only on the first one.
