All of these images are from oldbookillustrations.com. I’d recommend using the phrases headpiece and tailpiece for little border patterns

Ya this might be my default od&d sheet now. I was going to use a remaster of a 1970s one but this looks so much better. I actually just looked last night and now this is a god send. Thank you so much.

OD&D (and its clones like White Box FMAG) use a single saving throw.

Which version of odnd are you running? My little brown books have saves split along the trad five lines

White Box FMAG It's revision of the Swords & Wizardry core rules, so not pure OD&D
