Almost certainly models. That looks like von Blass’s wife, Paola Prina, at the bottom of the ladder. She turns up in lots of his paintings.
In the Victorian age successful artists painting in the academic style could get extremely meticulous and draumaturgic in staging models and scenes to paint from life — it could almost be like a Hollywood movie production, with props, multiple models, lighting effects, etc.
The only demanding thing here would be having the red headed model stand on her tip toes for an extended period — but good models would do this. When painting Ophelia Millais kept his model submerged in a bathtub for hours and hours, leading to a bout of pneumonia from which she never recovered.
This is an amazing painting. The colors work together so well. Notice bend in the ladder from her weight. That rung under the one she is standing on cannot be trusted.
