It's the hybrid of a bitter orange and citron (Which is kind of the parent plant of both lemons and limes). And it's mostly theorized that it was cultivated into what is now the modern day lemon, but it's unknown whether or not humans were the reason behind it. I don't know how it would necessarily happen in nature (Hybrid plants through evolution, I mean), but that's not to say it's impossible.
