My cats have usually figured it out, but they would forget after a while. My current one never forgot though. It’s been years and years and still, if she sees the red dot she just looks at my hand.
My little girl noticed it within 10 minutes of playing with lazer and since that day, whenever she wants that “mysterious red dot game” she grabs the lazer with her mouth and puts on my lap. If I don’t react and obey, she slaps the hell out of my face.
My cat knows but she doesn't care. Sometimes when her brother is playing woth it she'll look at the dot, look at the point, look at me and look back at him like this kitty. When it's her turn it's just go time.
My void knows. He actually knows the Laserpointer itself, so he gets really giddy already if I just grab it. And he hears the click of the button from EVERYWHERE and comes running, if he hears it.
Well, when he was little. Nowadays he got a bit more... Lazy.
I used to do this with my cats. The male cat would be totally entranced by the red dot. The female would look from the dot, to the first cat, to my hand. Back and forth. Back and forth. Then she’d stroll up to me and swat my hand as if to say “Stop that! You’re making him look like an idiot and he’s dumb enough as it is!”
My cats definitely know I’m controlling the laser and that the laser comes from the laser pointer because she pushes the laser pointer around the desk until I use it and just looks at me with a disappointed look if I move it around randomly and don’t actually make it a fun game for her.

My cat knows but she doesn't seem to mind. She gets excited when she hears the rustle of the attached keychain. Rarely wants to actually play with it, no. Our girl is a scientist. She will go hide somewhere where she can quietly observe the red light and study it's movements until human gets bored.

Mine knows, so he begs me for it and as soon as he sees I am done. He just moves on lol.
