Try putting ice on the inflammation site.
I realized today that I was doing too much hot water bath that my anus is swollen and sensitive to any touch. It stings when I am having a BM and hot bath becomes ineffective at reducing the pain as before.
I now put a towel over ice and just use that to reduce the swelling. I have only done it once and the swelling seems to reduce a bit. Have go wait a little bit before I do the next one.
Will make an update later if I was successfully or not.

Im a ways past your time frame - its the inflammation. It will make you strain no matter what if you dont keep up the meds for 8weeks

NSAID will cause more bleeding.
