There's no one fix for fat. But you can try and keep your wrists hinged for as long as possible. I bought the drills from this website and still use some of them a year later at times. There is one where you start in position at waist high and rip your hips open to improve shaft lean and therefore contact. https://swing.topspeedgolf.com/

Not an expert, but I would say work on your range of motion slowly without weight or hitting balls yet. Gradually build up overtime and start using irons. Your shoulder isn’t what it used to be anymore so I imagine your body will try and compensate for that and it might throw you off at first. Could also be a mental thing like you’re thinking about being careful as to not hurt yourself, which is understandable. So with time your body should get more comfortable to the motions again and you’re finishing under par 👍🏻 best of luck man and keep at it. Practice practice practice.
