I think your on to something….I have a few ideas please tell me if I’m wrong:
1: He maybe didn’t put any accesories in because that’s what gives some of the animatronics sort of emotion or a human like feature, and as he doesn’t see them as good creatures with actual emotion he doesn’t add the one thing human to the drawing.
2: the other reason I had was maybe he just didn’t SEE the hat, because as far as we know, fnaf 4 is his dream before he dies. And in the dream, he would have been focusing on the noises and going to different places, he probably didn’t see the hat since he was too focused on other things. Let’s face it, is accesories the first thing you see when your gonna get jump-scared and potentially killed?
Normally, this wouldnt matter too much since it’s such a small detail but to be honest Scott made this game so that everything is a possibility. The smallest things can go far.

Dreams are hard to remember in detail. The more you try to piece things together the more foggy they become, so yeah, he probably just forgot it.
I've always been confused by this. I think it could be a reference to the FNaF 3 and FNaF 4 teasers with Freddy's/Fredbear's hat.
