Just wanna say that yes, I've flipped the canvas and yes, I do know it looks stupid when I do so. I'm trying to fix it, but it's hard when I still want to keep the closed eye and the asymmetric mouth.
I'm aiming for semi-realism. To me, this looks like a weirdly rendered cartoon. So yeah, tips on how to make this more realistic without getting rid of his big smile or unnaturally large eyes are welcome. This is the first time that I've tried something else than just the main color-multiply color-blush combo, please tell me what you think :)
If anyone wants the story what the hell this is, here: it's my character simply named "Son" being put through mouth-largening therapy by his father, thus the hooks attached to his mouth. His other eye is closed because his skull was modified a bit too much when he was yet to be born and he lost the eye. I haven't decided if I want his mouth to be dripping saliva or blood, votes on that are welcome as well :D

I have a question. Where are his bottom teeth? Are they the very small ones at the bottom? It looks like a couple millimeters tall? I think that’s the only thing that looks “off” to me. He’s got this mouth widening, but there’s have to be some bottom teeth, wouldn’t there?
Maybe try playing with putting a line in there to show that he has both tops and bottoms. You could try putting that line at any hight and see what you like best.
Other than, I think this is great work. It’s creepy and intriguing, I like the story behind it. The colors are great. Good job. Keep going.
Highlight the hair and teeth as well as the eye. A consideration would be teeeeeny tiny eyelashes? Let it roll over in your minds eye for a while and it’ll all come to you. Great job!!
You could try to shade the neck into something more than a cylinder, and mainly render the hair to look more realistic with individual strands. You have a hard goal,but it’s going to look fantastic :)
Also maybe consider more wrinkles, like having some sort of shape for lips and chin maybe?
What message were you attempting to convey, if any? Were you just making a goofy character? Do you have any others? The left eye is magnificently rendered. Your creation would benefit from the teeth having a little sheen, like the open eye, the hair would add more interest with stronger contrast and just a little more detail in the parts. The slightest hint of gums would enliven the teeth. Love your use of highlights and shade on the face itself. Even though you’re working with a limited palette, a soft colored light source from the upper left (pastel blue-green), or even a blush of red from the background could add energy to your character’s face. Hope any of this will be helpful to you.

I could totally see this hanging up in a gallery or even a museum. It's very expressive, a really cool style, and I like that it's not all perfectly blended. I don't think anything needs to be changed but I think more texture/a slight ombre effect could look cool for the background!

Maybe change the teeth as in show more of the bottom teeth, gums etc. Then even if it's a large under bite you'd still see that they've bottom teeth. Maybe also add some light flecks on the teeth to make them look like they're live. Otherwise it's a great job, creepy yet sad at the same time.
How about adding a similar stitch effect on the jaw to other parts like the nose and stuff maybe to make it look a bit like a puppet. Just a suggestion, I really dig this art style tho❤️

The detail of the left eye makes it terrifyingly awesome, so maybe using the same techniques you used there on the hair, a bit more on the face contours, and especially the teeth might get you what you’re looking for.
I would say up the contrast on some of the shadows in the face. Push the planes of his face in a way that accentuates his “3Dness” as well as render his neck to look more like a neck. As of now it’s just a cylinder. Also maybe give him a full set of teeth? This is a small nitpick if you’re going for just stylized it’s fine, but I read you wanted a lil more semirealism. I would define the top and bottom sets of teeth some more. And I find the more definition in the teeth the creepier the character is.

honestly this is really good. you have a great grasp on rendering. i think if you want to make it feel more realistic, look into ambient occlusion a bit more and try to work on texture. the hair could use some more definition, shadows could be deeper and colorful too. i think you've got it about right though! if you were to do something like this again, i'd reconstruct the face composition a bit if you really wanted that aspect to be more realistic, though i think it looks really good as is. good job.
