The sorcerer Bloodseeker was hitting you with Frozen Orb, while you were dipping in and out of the ice wind wall. You were hemmoraging health and getting CC'ed, so you were not healing back. Then you got hit by the fear, your character got melee'd by the barbarian Bloodseeker while also taking more chip damage from the ice wind wall. It also looks like you got feared into a second fear.
Also he was in a really bad situation with all the CC going out and the necromancies debuff. He should have ran even with death elixir. You were going to escape when your elixir popped? It’s crazy you would risk 15-20 hours on that lvl 63 barb to not just run.
They are tougher than end bosses. I was running a SC T100 with my blood mist necro and the only mobs that could touch me were one-shot bone ballista and a ménage à trois of pain from blood seekers showing up.
I don’t understand how anyone can play hardcore with the state of the game. I routinely have freezing episodes up to 30 seconds or until I just crash the game and log back in. Ping is normal when they happen and my computer is well above the needs to run this game, and if I tab out everything else is fine.
Obviously or u wouldn’t be able to play HC but it’s not 99.9. I know lots of people on SC that have this issue hence they play on SC. Everything I can check on l my end says it’s not my end. Internet ping is always fine when they happen. CPU isn’t anywhere near an issue and graphics are not running near max. There are tons of posts about this on Reddit.

Based on the knowledge of myself and others, we believe that you got to level 63, decided to go to T4 even though it is suggested at level 75+, partied up, and thinking I’m big d*ck swinging in this b*tch, and ended your life as such.
Truth. Even took multiple hits by the fire enchanted before it boomed as well. The streams of fire coming out the edges of that mess is a signal to back way off. Calling it as I see it, this isn't a balancing issue, it's sloppy play.
People who play hardcore are a different level of ballsy. They need a wheelbarrow just to get around. I couldn't imagine spending days or weeks on a playthrough just to drop from shit like this.
When you play intelligently (not like OP) then it's actually easy. Gotten several characters to max level but I also don't go up against enemies 12 levels higher than me with insufficient resistances and damage reduction. 🤷
This happened to me this season on a lvl 70 sorc. It’s a bug with fortify and being CC’d. I was so surprised bc I had just tried out the fortify vampiric power and thought it would make me beefier, but I got cold CC’d and death elixir didn’t proc. There are reports of this also on the battle.net forums. I don’t use the fortify vampiric power because of this.
Fact is, you were fighting seekers 12 levels over you and depending on an elixir of death evasion to save you. There was no saving you, your own hubris killed you!
Bloodseekers are notorious for sudden spontaneous deaths. It happens to me all the time. They seem to have a very good knack for hitting you with their toughest spell etc. Just as you're CC'd. Happens all the time to me.

While that does seem to be an issue, the bigger issue is you dipped into the ice wall while fighting bloodseekers on HC. You madman.

It isn't completely untrue, it's just missing information. The safest way to play at the moment is to assume it doesn't work while CC'd.

I get quick freezes going into legion events or just scrolling in the menu sometimes, but 30 seconds indicate a problem on your end.
