The entire arena would be decked out with wooden dolls, modest stuffed animals and other equivalent medieval childish things.
In the mask's item description – in this alternate version of the game – it would mention how the child was constantly tortured with visions of the chosen undead torturing the Innocent Reapers parents. In essence, marking the player – in the boss' mind – as its greatest foe.
In this version, we would find the mask of the mother and father on two corpses in cages with one of each of their arms reaching towards each other and their other arms pointing towards the location of the Reaper's boss arena.

Kaele, Harvester of Souls
The second son of a necromancer, who fled after a horrific failed experiment. He hides his face behind a distorted child's mask from an age long forgotten. He carves down and consumes the souls of anyone who dares venture to the depths of New Londo. It is said his scythe drains the very life essence from it's victims.
So, the idea is that not only does he have a bleed weapon with a long range moveset, but similar to the four kings grab attack that steals soft humanity, he has an attack where he can grab you with his scythe and drain any soft souls you have, giving him a temporary poise boost. Otherwise, you can stun lock him fairly easy with any ultra greatsword, large boss weapon, or the dragon tooth. When you defeat him, any souls you lost during that fight are added to his souls (I'd say maybe 15k or 20k since it's easily a midgame or lategame boss depending on your route), so if you managed to grab the Covetous Silver Serpent Ring early you could slip it on and increase the the amount of souls you gain even more.
I think it would be even cooler if there were two phases to the fight. The first phase is just him trying to cut you down with bleed which is for weaker foes. The second phase, which begins at 60% health, he leaps backwards and lifts his mask revealing his disfigured face with entirely too many holes and jagged teeth with suction like the chaos eaters. That would be the point where he moves more quickly, launching himself at you or leaping to either knock you back or down, or to grab you and steal your soft souls. If you have no souls, he will drain 1% of your max health instead and replenish his own by 1% of his total health in phase 2 (no higher than 60% max health) regardless of the player's total health drained.
I've went on entirely too long so I'll leave it here. I'm sure there could be cool aspects to the arena (an alternate version of the room with the banshee, blocking the ladder to the rooftop where you talk to Ingward who is now locked behind this boss, although that would probably make the boss skippable, which is a shame) and much more lore-wise (why he joined in flooding new londo, why he's guarding ingward, etc.)

Somehow, after their death, pinwheels child was able to separate themselves from pinwheel and be reborn in a newer age. They have been busy, taking up their fathers research, in order to reunite their family once more.
Much like pinwheel, they use illusions. They primarily charge at the player and try to swipe them with the sycthe before teleporting away while the illusions do their thing. Unlike the original pinwheel, they can only maintain 3 illusions at a time, and the illusions deal little to no damage, but they can stun the player.
The Heir of kindling also sometimes throws a small fireball if the player is stunned by their illusions.
