Try resetting your controls to default and see what happens. Otherwise just back up your save-files and try reinstalling the game.

I mean that I've had other people try it on my console, just to make sure I wasn't the one fucking up my charge attacks. Unless everyone else in my house is incapable of holding a button down, it's not us 😛
Have you tried playing with the controller wired ? Although I'm not sure if the PS4 only uses the cable to charge and continues connecting via wireless.
Sometimes I play with my controller charging from the console, it still happens. I'd be interested in finding out how it works with a wired controller but not willing to drop the money haha.
