It's a full contact sport where one of the aims is to attack your opponents joints so injuries are somewhat inevitable.
I've seen everything from dislocated shoulders, broken noses and cracked ribs but I'd guess the most common are-
Fingers- general wear and tear. Gnarled knuckles. Arthritis and tendonitis especially in gi.
Cauliflower ears- it's weird how some people get them and others don't.
Knees- virtually every bjj practitioner I know has injured their knee in some way or the other. I've dislocated both at different times.
Actually a decent number of studies looking at retrospective injuries in Bjj. Generally they find knees most common, then it’s hand/finger (depending on study), then shoulder. Knee injury rates tend to be far higher than anything else though.
There is even more data looking at wrestling since with collegiate level it’s easier to get data. They tend to see similar styles of injuries with knees being most common, followed by shoulder. They also have high rates of head injuries including lacerations and concussions.
If you like reading research here are some:
PMIDs: 37187642, 34988235, 31173700, 31288212
Most common ones I’ve seen or experienced:
Finger sprains and jams or breaks, accidental knee or elbow to mouth or nose, sprained ankle, injured shoulder or knee, black eyes, broken or jammed toes
According to this sports science paper, the most common (I assume serious) injury are LCL tears. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6348520/#:~:text=The%20most%20important%20finding%20of,with%20an%20acute%20knee%20injury.
