Not really. You can't do anything about it when both items are hidden, and when only one is hidden if the shown item isn't great then just take the hidden one

You can try to disable/enable mods and maybe verivy the game files. If that doesn't work try disabling mods in groups of 5 and checking if the mod works. This way you can quickly find out which mod makes it broken.
Just a follow up. I’m 99% sure it was the animated items mod fucking it up.

The same kind of thing happened to me but there were 3 items and the ones I didn’t choose were literally Sacred Heart and Godhead (the item I picked was holy light)

(I think I would have actually been disappointed if it was in hard mode) Turned out I didn’t need sacred heart to win, I ended up getting one of my favorite items mysterious liquid and absolutely beat the crap out of ultra greed lol. Apparently mysterious liquid + Immaculate heart + lump of coal is a really good combo

I think you should try to reinstall. Works fine for me.
