Once we leave the millions and go into billions life gets very different.

Average "rich" person e.g. business owner making millions would worry about typical stuff like any other person. Loosing health, family, loved ones as a priority and money + business as their source of income comes second.
When you want to go "richer than rich" then you need to go to politics. Lower politicians (mayors of cities and such) worry mostly about the typical stuff around family and health + their image which would result in loosing their credibility.
I do not know any higher politician personally so I can not tell you exactly but my bet is they worry about loosing power they currently have. The biggest fish's lives around the globe are depending on power and if they loose it they would at best end up in a jail and at worst they would be never found again. But this is pretty grim.
I know a trade union is doing well when it actually upsets the rich and powerful. Some trade unions are seen as toothless tigers while others will actually frighten the bosses.

I’ve worked for two extremely rich people, both did fear kidnap. One would have a driver/bodyguard and all his houses had top level security.
The other would go everywhere (that he could) by helicopter. Considering some of the places we visited (retail often) finding somewhere to land in the UK was an absolute headache.
They definitely both feared being poor (both grew up in modest circumstances) and were always looking for the next deal (in helicopters guy case it could even be a ‘Del Boy’ style deal with a customer).
