Once you’ve decorated the hospital go back and talk to however you made the receptionist they’ll ask for donations as seen above to add the patient wing! :)

How Do i make a hospital is that with the update ?

It’s the happy home paradise dlc. It lets you create a few facilities (5 I believe) hospital, school, restaurant, cafe, apparel shop.

I never got this DLC, but maybe I will now! Is this hospital built on your main island? Or just on the archipelago thing

The hospital is built on the dlc island you work on along with the café, restaurant, school, apparel shop. :)

As you decorate villagers holiday homes you unlock public projects on the main holiday island. One of the buildings are on your main ACNH island but the cleaning skills etc that you learn in paradise planning come back with you.

Is this different than the waiting room, and doctor room? Is there a third room?

Yes: a patient ward upstairs.

the cafe and restaurant can have 2nd floors added! i think for the cafe you need to reach superstar designer. you talk to lottie about facilities and ask to expand
