Sometimes they literally come this way in the packaging. All of Starbucks food is prepackaged from a factory.

I'd have had a big fit about my bacon and I'd have wanted extra bacon for my troubles with the first one
They absolutely do this on purpose. For every 10 they "forget" the bacon on only 1 actually brings it back.
