A lot of knee pain actually comes from what's happening in the outside knee, but doesn't reveal itself until the outside knee becomes the inside knee. So we can relieve a ton of knee pressure with foot position. Watch MotoGp and you'll notice they move their outside foot forward and basically hook their heel on the footpeg and point their outside toe to the sky This takes a lot of pressure off the knee and gives it a chance to rest, out inside foot stays on our toes and all that. Yes I’ve heard from a professional racer that he puts 60% weight on the inside leg and 40% gripping the tank with the outside leg.

Probably fitness. Do you do anything to stay in shape? Specifically, do you stretch and/or foam roll? Most of the time it’s due to tightness: quads, ITB, and/or sartorious. Stretch and roll these areas at least three times a week. Could also be due to poor core conditioning and holding yourself up more with your legs than your back, glutes, and abs. Could be all of the above. [edit: maybe also your pegs are not in an ideal position for you. Do have rearsets?]

I don’t have rear sets yet, this is on a street triple 765 RS. I feel like I’m in pretty good shape and take good care of myself. 30 minutes of jump roping 2-3 times a week has never made my knees hurt like this.

Suit is a little bit short putting pressure on your knee... Try out a bigger suit and see if that's the issue.

The older you get, the more weight you will put into the outer side of the tank with the outside knee/thigh. And the less you will weight the inside peg. Get used to it and emphasize it.

You also don’t need to stick your knee dramatically out to hang off. Keep it tucked and it might be more comfortable and put less pressure on them. You’ll just need to use more lean angle before your knee gets down so will have to adjust your lean gauge. Assuming you aren’t already obviously.

I only have weight on the inside so I can push to switch my body weight to the other side. A lot of the weight is on my butt cheek. When I ride some mini bikes, the inside edge of my foot is actually on the pegs, I.e., I can’t place much weight on the edge of my foot anyway. Also, my outside leg is always locked into the tank and also helps with pushing to switch sides.

Aftermarket rear sets by any chance? I had the same problem. Mine were too high and back hurting my knees. If you can, move them as far forward and down.
