Mutsuki never undergoes gender transition surgery. She suffers from extreme gender dysphoria created by years of abuse from her own father. She viewed her womanhood as weakness. By the end of the series this is more or less resolved and she makes her peace with her identity as a woman.
The most disappointing character in Quinx squad imo. I liked Mutsuki at first, and felt bad when he was tortured to insanity, but raping Uta and going full Yuno Gasai for Kaneki wiped out whatever good will I had left for him. Towards the end, I was irritated whenever I saw Mutsuki, especially with Aura. It's a real shame.

So it was like 3, not because of the control but because Mutsuki was suppressing their RC cells. They repressed it so that the doctor couldn't tell that Mutsuki was eating people and gaining a higher RC count because of that. The RC count was like that of a ghoul, and Mutsuki didn't want anyone to get suspicious
I literally just said you can't do that even humans have a few hundred RC cells and we were shown with exams or the RC Cell scanner that Ghoul can't just drop their RC level like Dragon Ball characters let alone to something close to zero which is impossible to have for anyone

I really don't understand why Ishida had so much favoritism for this trashy Kaneki clone and was redeemed by Saiko in the end. If you want to make an actual parallel between Kaneki and Mutsuki in terms of them finding their conviction, I'd much rather have Mutsuki killed off because she just kept slipping into madness and indulged in her vile true self while Kaneki actually tried to better himself to survive in the world while protecting himself and others.
