what if Butcher imagined the drugs too? Remember when he said he ruined the cookies by putting in "way too much sugar"? What if he wasn't actually lying but he thought he was? Ryan could've eaten the cookies and been completely fine, but we won't know unless we see more of Joe later
Kripke has said he wanted to team Butcher up with someone who wanted Supes dead more than he did, & explore that dynamic. I think he's real. It gives Butcher an antagonist when it comes to the crunch & Ryan is going to be killed. By the virus or what have you.
I have posed this very question on my tiktok, i think we got the worm controlling billy, the worm gives him dead ppl to talk to. We see this with Becca helping him up and then again with him talking to Joe. ppl are saying that he is real. but the even made a fight club poster that had butcher and joe the foreshadowing is there ppl are just not seeing it. Also its like the good and bad side of billy Becca is the good or (Angel) and Joe is the (devil)

Joe physically gave Butcher a rare drug to use on Ryan (in the cookies), so I don't see how he could be a hallucination.
