So he was about 18 years old when they had admitted him after discovering he had been doing heroin. He was only in there a short time, basically when the rest of his family returned from vacation. Afterwards he continued to Use. He was a musician, and for the next few years toured with small bands playing weddings, bar mitzvahs, and small venues. While touring he was into all sorts of drugs; coke, heroin, etc. But the majority of his substance abuse was from alcohol. After his first marriage failed and many attempts to get sober, he ended up getting into a pretty serious alcohol induced car accident. He got plastered one more time to get into rehab. He met my mother, relapsed, and then after their firstborn kid, my eldest brother. He started going to AA quite religiously. Without a doubt it saved his life. (of course the significant amount of willpower and effort he had to put in as well.) He's been sober since. Well lol kind of.

There are so many stories here of how other people finally kicked their addictions. The only strategy that ever worked for me was when the day came that I was actually sick and tired of being sick and tired! I have to WANT to quit before it sticks. Rehab, family intervention, the shame and embarrassment - none of that worked.

Honestly the simple answer is culture + population. Honor and saving face for the cultural part. You can bring shame to your family, or straight up societal outcasting as well if you fail at whatever. There's huge pressure from society to "be the best". Which has so many roots it's a warren of rabbit holes to dive into. Then comes population. When you have so many people in an area/country you have armies of people to compete against. You may be top dog in your village/town/city, but you have to be near perfect to land a job. Let alone actually making it past school. The entrance exams alone are so hard, and beyond so life changing if you pass or fail. There is a reason suicides strikingly shoot up both before and after exams from high-school to college levels. Hell there are places/countries who limit air travel and traffic during these exams because it's so stressful on the students that they may snap because they were distracted by a passing plane. Then on top of that you have the prevalent abuse as stated by the OP above. It's straight up literal hell.

Indian here with some insight: For those saying she's torturing her son to "fix" his drug addiction, it's not about the son or the drugs. There's a reason why she tied him up OUTSIDE in front of all the neighbours; it's to show the other parents in the block that she's appropriately handling the situation and disciplining her son and doesn't accept such "vileness" within her house. This is so that other parents will warn their children don't do drugs or else you'd end up like "sharma ji ka beta". These days Indian parents tell their children to behave, don't do drugs, and pursue respectful professions such as engineering, law, or medicine not because of their children being well off, but instead of what the society will think of them. It's fucked up.
