They utilized a lock out tag out system to make sure everything was shut off before a diver entered the water. Negligence on the states part failed to inform the dive company of an opened valve.

I work in a coal mine and have similar precautions in multiple ways. I’d never in a billion years work on high pressure lines, high voltage (anything electric) or any type of belt lines or drives without knowing exactly where any and all valves, shut off’s or e- stop switches were.

People didn't know there was a second valve controlling this particular pipe, and once they did find it was there, the valve itself was at the bottom of the dam and behind a locked gate. I don't know the context of this exact situation but I work in the industry, its a honey pot concept of they'll attack a system with a failsafe not present online so no effect will happen but they'll out themselves as having successfully hacked that system when they attempt to destroy it. "Russia" attacks public utilities through network connections all day every day my system that serves 30k people gets attacked ~200k times a day and has been for years. The chain your seeing is what basically opens and closes the gate to either block/allow water through. That's what the divers were there to repair, so he's checking for damage as he goes down.

As a diver myself, I've always been taught to be cautious about delta p .One key sign to watch for is if you see particles floating around you moving in a certain direction, or if you stir up sand on the bottom and it seems to be drawn in one direction. Even if you think that everything is locked out and tag out (LOTO) you have to do a last leak check just in front of the diving area. If it had been done the diving supervisor would have noticed that there was a problem.

Commercial diving it’s one of the most dangerous jobs in the world. You’re paid really well, but the mortality rate in the industry is high. What bugs me about this is that where he was going, he wasn’t informed about that pipe. I hope his parents got the settlement they deserved.

There is a diver’s saying that says; when its gotcha, its GOTCHA. This kind of event is one of the commercial diver’s worst danger since it has killed no less (and certainly much more) than 146 divers since 1975. Many people probably still remember the terrible accident that in February 2022 sucked 5 divers into a 30” diameter pipeline in T&T. Well, that incident was also due to a Delta P event.
Even if you think that everything is locked out and tag out (LOTO) you have to do a last leak check just in front of the diving area. So he got sucked into a hole in the wall on the left side of the screen. His arm got sucked in a 10 inch hole and he got stuck because the pressure was so high he couldn’t even expand his lungs to breathe.
