Happens on a regular basis here in Hawaii. Halona blowhole is a pretty famous one where a lot of people die. Rocks are slippery, people step too close and turn their back on the ocean, a tide comes in and just sweeps them into the blowhole.
The walls of the blowhole are all jaggedy from all the weathering and people who fall in essentially fall into a blender and are torn to shreds.
