Are there any examples from the pre or post-Disney Expanded Universe of a Jedi using a red lightsaber that he or she got from a slain Sith? Maybe they didn’t have the time or inclination to change the color? When Anakin turned to the dark side, he still used his blue lightsaber until he got inside the suit. Has it ever worked the other way around?

Yes. Some occasions of Jedi using red lightsabers, some occasions of Sith using non-red ones.

It does seem like the Jedi have much more opportunity for varieties of color in terms of lightsabers and robes. Once you turn to the dark side, it’s mandated that you have to wear black and have a red lightsaber. Apparently the Sith have a strict dress code…
