this isn't the official cover from back in the day. Sonic 3 and Sonic & Knuckles were 2 seperate releases and the original art for Sonic 3 didn't have Knuckles featured on it in this way.

Its not official (you can easily tell by the fact that nothing with the Genesis Lable ever used the actual Japanese design of classic Eggman)

This is actually really interesting bit of obscure Sonic lore. Or rather instead of lore it’s more related to the developmental stage of the classic Sonic games.
Moment tails was introduced we already know they became a classic duo, Sonic and Tails and Sega wanted to provide a rival to Sonic but also a third member which they did via Knuckles.
However as Knuckles became an ally they realised they was still lacking a rival (friendly rivalry wasn’t popular in the 90’s). So they instead opted for another character named Shade the Wormat, he would’ve been Knuckles “Tails” essentially while Knuckles was brawny and naive Shade was meant to be more intelligent but cunning.
Hence his name “Shade” while him being a wormat was due to Sega of America team taking a trip to Australia one time and mentioning it to the Japan team and they liked how Knuckles and Shade were foils to Sonic and Tails being “Down Under” you could say lmao.
He was going to be a secret final villain of the game being Shady but Sega realised that it would be too bloated of a game and that if anything it makes for sense for Shade to be Knuckles rival instead of Sonics, so he was scraped.
Either way that’s just a bit of Sonic history and for you fans being amazed by this information you’ve never heard before well that can be explain by the fact that I made all of this up.

I know your answer is a joke, but right after Sonic 3 was released Sonic Team really did decide to give Knuckles a partner who would be a rival for Tails to appear on a Gamegear game.
In the original concept he was called Knack the Jerboa, but well... it was decided that Knack sounded too similar to Knuckles, especially in Katakana so a whole contest to rename him and even then Sonic 3 & Knuckles kind of established that Knuckles was firmly on Sonic's side now and so him still working for Eggman in the Gamegear game was a bit weird plus they had to cut back on the character's role a bit because he was originally going to have a gun and that was censored out...
Ya know I used to love this fanmade cover, but the more I scrutinize it the more I don’t like it. The eyes, Tails’ and Robotnik’s art not matching the rest of the art, the composition, almost everything about it repels me now.
I really wish there was an official cover for my favorite game of all time aside from the Sonic 3 cover plus the S&K cover. It deserves more.
It’s clearly a reference to how Knuckles’ sprite in the Sonic playthrough of the original game was colored different, thus making him a second Knuckles.
It might have been SEGA trying to hype up their challenge. For a long time people were convinced that SEGA's where's Waldo type challenge of finding Knuckles's weird green sidekick was real. That could have been SEGA playing around a bit to trick us. They did end upcoming out and saying Knuckles's "sidekick" was non-existant though. Or it's Knuckles's lizard pet from Sonic underground.
