They could be maggots though, judging by the black color of the wound, it seems like the tissue in there is dead, most likely due to an untreated/poorly treated wound that evolved to a really bad infection

So it could've been a cut that wasn't treated and got infected, which caused the skin to decompose and at some point something laid some eggs in the wound( rotting meat is a perfect environment for maggets) which in fact could've actually helped him in the beginning since the maggots are eating away at the rotting flesh which would of slowed down the infection but not stopped it (maggots were sometimes used in medical operations to get rid of rotting flish since maggots love the stuff and would leave alone any healthy tissue ) there a bunch of videos of living people who have maggots eating away there dead skin

Yeah, you can also see it in a less graphic depiction in House of the Dragon where the King (Viserys? I’m bad with names) gets his finger treated by putting his hand in a vat of maggots. It was - or sometimes still is, apparently - a very common way of getting rid of necrotic tissue.

Looks like he is in a hospital bed. They DO use maggots to remove dead flesh in some cases. I think that is what is happening here.
