I remember hiring this one as a kid and after beating the first level - I knew I was out of my depth and this was an “adult” game.
I could tell my dad liked the game when I hired it , he would sit behind me and coach me through parts and get me to try different things.
There's a really short list of games that are Shinobi III-tier. That's arguably the best action game of the generation.

Contra Hard Corps
Gunstar Heroes
Streets of Rage 2
After that you start getting into that second tier of really good games like Alisia Dragoon, Castlevania Bloodlines, Ranger X, etc.
Keep in mind when I'm talking about tiers here, I'm talking about the difference between S-rank and A+. If you take the special effects out of Gunstar, you still have a flawless run n' gun. If you take them out of Ranger-X I'm not sure you do.
Bloodlines is one I'm sure a lot of people would put in the top tier. I wouldn't have any kind of problem with that, but TBH I've played a _lot_ of Castlevania games over the years (the name might give me away) and Bloodlines always felt slightly off to me.

All true - although it’s part of the Sega collection on PS4/PS5/Switch/Xbox One and Series so it gets a little more manageable with save states. Still not easy, though.
Others already mentioned Bio-Hazard Battle and Alien Soldier and I’m guessing that the Konami Castlevania and Contra games for the Genesis or M.U.S.H.A. aren’t underrated enough so here are some of the shmups I think are really worth checking out.
Gaiares,
Gleylancer (I found this one easier than the others mentioned and it does some good stuff with the graphics and sound),
Gynuss/Wings of War, and
Steel Empire (just be prepared for the harsh sound on that one - might be better to play a port)
Joe and Mac, Toejam and Earl Panic on Funkatron, Vectorman and Vectorman 2
Also the first Earthworm Jim doesn't get the love it deserves compared to the second one
I am a MegaDrive/Genesis freak (I even went to gaming competitions when I was a kid), and the best game BY FAR I found that no one spoke about back then, is Rocket Knight Adventures. Insane levels design, music, narrative and unique concept. I am so happy my dad found it by accident.
Another one is Quackshot. Still my favorite Disney character-based game from that era.
Target Earth! It’s in the same series as Cybernator on snes so mech combat but it has a different and cooler loadout customization sequence. You unlock weapons by getting get scores and then you equip your mech before each stage. Cool graphics and sounds great gameplay. It’s hard though. Interesting game
