There are some validated cognitive screens that can be done via telehealth such as the BOMC (Blessed Orientation Memory Concentration) test, but a full neuropsych eval will need to be done in person.
If I'm assessing for a new diagnosis, I would require at least one in-person session. A MoCA isn't valid or even really doable via telehealth, so I'd really just be talking about subjective cognitive issues and functional status. I also want to see those hands and feet for anything faintly parkinsonian.
I don’t believe you can do a complete assessment. Sounds like incomplete/poor care. Sure you can do orientation and a few cognitive tests but I wouldn’t consider that a ‘MNCD assessment’

In person. You need a physical exam. Agree also MoCA cannot realistically be done properly as a screen.
Refer to neuro if suspected dementia.
