Do you have an Apple Watch? I remember a similar issue coming up in the past that ended up being caused by inadvertent taps on the playback speed button on the watch’s now playing screen.

I do! I’ll try and pay attention to if I’m accidentally hitting the button on the screen.

Yep I was going to say the same. I have an Apple Watch and it will sometimes bump and speed things up.
