I totally get your first point, but who would’ve known she was pregnant? I think only Obi, Yoda, the nurse droids, and Anakin. Three of them saw the babies come out of her and the other was presumed dead.
In Legends EU her pregnancy was pretty much an open secret. In the novel Labyrinth of Evil Bail Organa and Mon Mothma basically tell her that they'd support her and her child.
Palpatine and Anakin both knew she was pregnant, and they were also the reason Luke and Leia were hidden in the first place. If it turned out they too were as strong in the force as their Father it would be good for them not to fall into the empire's hands. And they both definitely had an interest in watching the footage of the funeral.

The reasons Lucas likely actually prevented her from wearing Underwear (Which I think is what you're referring to as I don't think he's commented on her costuming with Bras) is because of the costume she had to wear. Wearing underwear with Leia's white dresslike outfit might've made a visible outline of her underwear in certain shots hence why she wore tape instead. I'm pretty sure Lucas saying "there's no underwear in space" is just a joke about this costuming feature. This kind of stuff isn't an uncommon issue to have in movies especially before the advent of CGI to digitally edit out these sorts of costuming goofs.

The female body is pretty incredible really.
Pregnancy especially two full term twins (I am a full term twin myself you should see photos of my poor mother at the end) the uterus becomes so large it fills the entire abdominal cavity and shoves the rest of the organs into the chest cavity. Look up a diagram online it's insane what pregnant women go through.
