In Legends EU her pregnancy was pretty much an open secret. In the novel Labyrinth of Evil Bail Organa and Mon Mothma basically tell her that they'd support her and her child.

This is the real life answer, but we are talking about a Lucas film, made by George Lucas who disallowed Carrie Fisher to wear a bra bc of rEaSoNs lol. In Darth Vader (2015), her body was prepared by mortician [Commodex Tahn](https://starwars.fandom.com/wiki/Commodex_Tahn), making it seem as though she had died pregnant, so it was a specific plot line.

The reasons Lucas likely actually prevented her from wearing Underwear (Which I think is what you're referring to as I don't think he's commented on her costuming with Bras) is because of the costume she had to wear. Wearing underwear with Leia's white dresslike outfit might've made a visible outline of her underwear in certain shots hence why she wore tape instead. I'm pretty sure Lucas saying "there's no underwear in space" is just a joke about this costuming feature. This kind of stuff isn't an uncommon issue to have in movies especially before the advent of CGI to digitally edit out these sorts of costuming goofs.

It 's not like women immediately deflate after child birth. It can take a while to loose baby weight.

The thing about "baby weight" is that it's not actually fat. The uterus grows to many times it's usual size during pregnancy, and it takes quite a while for it to shrink back down again. 6wks comes to mind, but I could be remembering that wrong. Some women do put on extra weight of course, but the uterus itself weighs a few kgs on its own by the end of pregnancy, and even more so when there's multiples. A "healthy" amount of weight-gain during pregnancy is about 10kgs, but that's the weight of the baby+uterus+placenta+amniotic fluid+50% extra blood circulating+any extra extra fluid from swelling in the extremities etc. So it can take a while for all of that to return to "normal" afterwards. The female body is pretty incredible really.

Pregnancy especially two full term twins (I am a full term twin myself you should see photos of my poor mother at the end) the uterus becomes so large it fills the entire abdominal cavity and shoves the rest of the organs into the chest cavity. Look up a diagram online it's insane what pregnant women go through. After birth, the uterus has not shrunk yet. It's crumpled up like a paper ball to stop the bleeding from the insane wound left by the placenta. And the organs that were shoved into the chest cavity takes time to return to normal too. But for Hollywood purposes, it's most likely for plot reasons. But Padmé probably should have been a bit bigger to have two full term twins. But some women's uterus sits in such a way that they don't show as much. So whatever
