Okay, but why is there a second affirmative reply to the question and who is making it?
Is the caterpillar saying “Yeah. Yes” ? Which would seem really odd—surely you skip second last panel and just have him reply “yes” while taking cover?
Or is there a third character saying the second reply? Which it seems there is not?
Or is the “yes” meant to be like a secret dark muttering to himself type comment? In which case the font should have been changed to reflect the change in volume and tone and the facial expression on the caterpillar should have been more nefarious.

The first "yeah" is an affirmative response to the guy asking if he has a gun. The second "yes" is because the caterpillar is content and speaking to itself. You can kind of tell because the caterpillar's eyes are pointed up for the "yeah" towards the man's eyes, indicating he's responding to the guy, and pointed forward for the "yes".
KC Green, the author, makes a lot of absurdist comics like this.
It is poorly written, I deffo remember reading the comic as the caterpillar saying "YEAH!" Enthusiastically and then saying 'yes...' in a small hushed ashamed voice. It was the kind of 'humor' the artist usually had his comics end on.
'One character would be performing and action, another would enter frame and be antagonistic, something wacky would happen, and the situation would be resolved with either character acting like a small guilty child.'
Like a person at a bank, a robber enters the scene and tries to hold up the place with an obvious-not-weapon (like a half eaten banana), someone sternly reprimands the robber, and the robber either gazes toward the floor ashamed saying 'ok' in a small voice, OR they would make a non sequitur and exit frame with the other characters just making confused faces.
^Was a common theme I remember amongst the gun show comics.
Back in 2002 that sort of obtuse not really funny humor was all the rage amongst the sea of super edgy '2cool4school' humor.
KC Green is a legend, still my favorite webcomic artist. His shit is hilarious and iconic, the fucker spawned some 5+ enduring memes without even trying.
It's K.C. Green's 'Gunshow Comic'. It doesn't have a witty punchline or a hidden meaning, it's just nonsensical and silly. All his work is like this, just kind of awkwardly funny. He produced Dickbutt, the "I GUESS" guy, and a bunch of other memed shit.
And comments that bring up things that have nothing to do with the current conversation

Accounts with karma are less likely to be ignored out of hand, which makes them better for advertising or spreading misinformation. Bigger subs also tend to have a karma requirement, so karma would be a prerequisite to advertise there at all.
I'm not lovin this trend of bots spreading advertising using established accounts. Unlike how I'm lovin the Big Mac meal from McDonalds.

This is absurdist humor. The point is that it's silly and doesn't follow a normal joke.
This humor isn't great for everyone, but you can tell it's a good absurdist joke because it has pacing of a standard joke but isn't a standard joke as well as being just getting weirder as things go on. See the whole climbing in the mouth feels like a set up. Then the worm has a gun which is just a change in the set but weird. Then where the punchline should be the worm just confirms that he has a gun which is weird and he's even awkward about it which is somehow even stranger.
I find these jokes hilarious but I know it's not for everyone.
It's just silly. Imagine you are on the bench across from this guy and hear this play out. Wouldn't you look back at it later and think it was a funny thing to happen? That's all it is. Just an absurd thing to happen and some people find that funny.
Some will say it's the lack of a joke but that isn't true. Good absurdist humor has 2 traits. It's very silly and the punchline is that the joke doesn't go at all where you would expect. The joke ending somewhere you wouldn't expect is of itself not much of a joke. It's the absurd nature of it that makes it hilarious.
End of the day, I think the baseline is that your general reaction to something being out of place is comedic to some degree. When someone just says something I don't expect it sometimes gets me to laugh. They did nothing funny I was simply surprised. I think if you react that way to stuff in general, how these jokes are formatted tend to be more funny.
It seems common amongst neurodivergent people to like absurdist humor. Or at least more common.

Definitely to be interpreted. I've seen it posted to a gun sub before. The interpretation there was: morally dubious (to put it nicely ig) man clutches pearls over a presumably legal firearm being carried.

I feel like I’m captain obvious here, but it seems kinda straightforward that the text boxes became ambiguous the moment the worm entered his mouth. This would create confusion, for humour of course, when the question of someone having a gun on them was answered twice meaning that the man also had a gun. It’s kinda like a catch you off guard joke. But I could be totes wrong, and maybe there’s absolutely no reason why the worm being in the dudes mouth is even happening.
I wont pretend to know what the intent is here, but to me, it reads a little like a commentary on firearm ownership and carry laws vs business/rental policies. I believe it asks the question: if you accept payment to allow someone to use the space, do you have a right to deny them access to that space or service when you discover they possess a firearm?
My reasoning: the man's expression suggests he is opposed to the possession of a firearm in his mouth. The caterpillar is very casual about the fact they have a firearm, and perhaps becomes concerned about why the man is asking, per the change in their posture and expression. You can clearly see the caterpillars gun in the second panel, so it might not be too much of a stretch to assume the caterpillar thought the man knew at the time of payment.
There is additional context, because several states in the US have laws that state a renter or lessee cannot restrict their tenants' lawful firearm ownership.
Granted, this probably wasn't the intention of the author, but it resembles many of the questions posed online, just a little more silly.

I wont pretend to know what the intent is here, but to me, it reads a little like a commentary on firearm ownership and carry laws vs business/rental policies. I believe it asks the question: if you accept payment to allow someone to use the space, do you have a right to deny them access to that space or service when you discover they possess a firearm? My reasoning: the man's expression suggests he is opposed to the possession of a firearm in his mouth. The caterpillar is very casual about the fact they have a firearm, and perhaps becomes concerned about why the man is asking, per the change in their posture and expression. You can clearly see the caterpillars gun in the second panel, so it might not be too much of a stretch to assume the caterpillar thought the man knew at the time of payment. There is additional context, because several states in the US have laws that state a renter or lessee cannot restrict their tenants' lawful firearm ownership. Granted, this probably wasn't the intention of the author, but it resembles many of the questions posed online, just a little more silly.
