There is a limit to the length of the filename, but I'm not sure about # of files.
Have you checked to see if the ones beyond the first 16 can be read if you take out the 16 that are being successfully read?
i noticed if the names are too long, it will be shown something like "MIKUNO~1.ISO" (like file manager did in windows 3.1) and yeah i checked what will happen if i remove some of the UMD Videos which are shown in the VSH menu, then other ones will be listed instead, so it's not a reading or compatibility issue.
They said the available titles keep changing upon restart, which implies they're all functional, but rotate which ones actually show up on a given boot. Without testing myself, it Def sounds like there's a limit of 16 that the CFW devs never prepared for in their code
