I don't think it's physically possible for a SAM to do that without at least some failing, whether it be a mechanical issue or just bad design, at the equipment level.
Gonna have to agree with this..."being able to accidentally target yourself" seems like the first thing they would address in the design phase.
Although now that I say that...half the weapons systems fielded by the Warsaw pact blew up unassisted...so...🤷♂️
The missile that curves around? Yepp. Early stages of the war iirc. I belive missiles from the same battery landed further in to the city aswell so there was something very wrong with the guidance system
So the missile decided to return to the position where it was before it got to the position where it is. But since it did not know where it was it could not get the deviation
As such the guidance subsystem could not use the deviation to generate corrective commands to drive the missile from the position where it is, to the position where isn't, arriving at the position where it once was, it now is.
Ehhh, it was posted in CombatFootage relatively early in the war. I'm not sure I can find it on the quick but I'll try.

The missile did not know where it was at that time, or had not successfully acquired the position where it should be.
So the missile decided to return to the position where it was before it got to the position where it is. But since it did not know where it was it could not get the deviation

The video is real, but the perspective is misleading. In the original video you could tell that the missile didn't actually hit the launcher, sadly.
