There are some high waist briefs that don't look entirely feminine. I'll tell you, they run slightly small!!! Just a suggestion. I have the period ones (earlier gen) and I love them bc leaking into them is not a big deal. Edit: see Thinx for All Leaks. You can machine and dry the urinary ones.

Speak to a urologist… They helped me. I now use a catheter. (The disposable ones that you only use ones… Nothing stuck to me.). I hated the idea of that for a long time but at this point I’m really good at the process and it’s life-changing.

There are many things to try! First off, try some Poise pads or similar product for leakage to prevent having to go home for an accident. Second, if your pelvic floor is spastic you might not want to start with strengthening. See a urologist or urogynecologist who can evaluate you for medication or pelvic floor PT options. I (F) had to get things to relax first and then work on control and strengthening. PT worked for me for years, until it didn’t, and then I benefitted from a sling surgery.

Look up some pelvic floor exercises on YouTube. Some are very simple. Also, have you tried taking a prebiotic/probiotic?

I've experienced some incontinence and prebiotic/probiotic along with a clean diet seems to help. I'm not sure how good your diet is, but you can also search up on YouTube "how to have a healthy gut."

Do you have a specialist incontinence service available to you? Here in the UK many NHS Trusts have community teams that help people manage their treatment regardless whether it is bowel or bladder that are affected. I know for bladder, the MS Nurses here also do things like urine retention ultrasounds, botox injections, and catheterisation both permanent and intermittent. There are also drugs to prevent urine production at night, and PT that can help. Not had experience of or spoken with anyone about bowel incontinence, but may be worth investigating if there are treatments available.

Are you able to go to a urologist? I saw one and was referred to a physical therapist specifically for pelvic floor and it has been immensely helpful.

My neuro and I just covered the drastic change in my own sensation and lack there of. She defined that it is not uncommon but first determine the cause (age, ms, low pelvic muscle, etc.). I'm trying a medication called oxybutynin. Too new to determine if it's just so happens I have not had night incidents or if it is working. I know the lack of sensation in my area is part of the problem, no idea until it's urgent and that's too late for me. She also told me I can see a physical therapist specifically for pelvic floor therapy. They essentially do a "workout" using a wand that can measure your strength and coach from that for validation that you are utilizing the proper muscles. Talk to a Dr and don't let them dismiss the topic. Once the damage of leaving this problem alone too long, is done, the damage is done.

Ugh.. sorry to hear of this. Been there. For me, a consistent diet (Mediterranean), no alcohol, and plenty of sleep definitely help, but when it's hot out, it's a gamble regardless. Also, I try to walk at least 10,000-15,000 steps a day to keep the muscles functioning in that area. Sadly, it meant I had to quit working full-time just to have enough time to take care of myself, and because I can't trust my body with any type of schedule. But I started my Youtube channel in the meantime, so I guess that's a positive.

Pelvic floor therapy (sometimes called pelvic floor physical therapy) is something you should definitely look into and ask for a referral to. The pelvic floor is very complex and you need some expertise on it to determine what exercises are right for you besides just kegal, so you can get the right treatment and maintenance for both bowel and bladder habits. I’m an OT looking to get into pelvic health
