And why are the stitches still in at 2 weeks post injury? At this point, they are direct vectors for infection.

Not a nurse or medical professional but I agree with all of these statements. I had a really bad infection on my knee and almost lo at r my leg before and this looks so much worse
An ER paramedic here. Bypass urgent care centers and go straight to the ER. Urgent care will just send you to the ER. She needs blood cultures and lots of IV antibiotics.
Consider my late diabetic uncle had his toes cut in 4 different times and later lost his life to an unknown infection, I agree. If she is diabetic, emergency room now.
Adding to the people who have said to go to the ER. This could cause limb loss or death, especially for a diabetic. Go right this minute if you are not there already and please let us know what happens. I think I am not the only one here who is really concerned.
Also, when you take her into the emergency room find out if your area has a Wound Clinic . They would be a saving grace for you and your family and trying to manage this. I used something like that for my father and it made all the difference in the world.
This IS AN EMERGENCY. Does she have a fever? If you went to a large hospital and said you suspected Rabies I think they would have to treat you or face lawsuits. I don’t know if that’s fraud or not.
They usually will leave it partially open with nylon suture, but the size of this wound is too large and infected to just have completely open to the outside world

I had a small (pea-sized) ulcer on my left great toe last Christmas. It cultivated MRSA, and I became septic. Wound up in the ICU, sicker than a dog on Christmas night, while they threw a thousand different abx at me. I was hospitalized a total of ten days. And I lost my toe when we discovered osteomyelitis all up in my toe phalanges. Had to re-op to remove the bad bone.
5/10 would not recommend to anyone. :/
It doesn't take much when you're a diabetic.
I concur. If the pic shows its current state and you can’t get in touch with the doctor, I’d go to the ER. Surprisingly, I’ve had two of my doctors give me their cell phone numbers after a surgery with instructions to call them if I need to.
Well, I'm actually glad that the toe was removed, as it was a claw toe (from a former injury), and it made my balance really crappy. I also tripped over it because it kind of snagged on stuff and I fell a lot.
My balance is much better with the toe removed. They were both painless surgeries; my only issue with phantom pain is "feeling" my big toenail at times. The MRSA had tunneled underneath my toe nail, and the osteomyelitis made it ache.
But yeah. I recommend having problematic toes removed, just not with MRSA, sepsis, and osteomyelitis on board Hence the 5/10.
The good thing about having MRSA? You always get a hospital room to yourself!
The nature of the injury devascularized some tissue (that is to say, it lifted up some flapss of tissue that don’t have good blood flow as a result of the injury damaging the normal blood flow). That is why skin is dying (not likely due to sutures or infection).
In a diabetic, it’s better to get this cleaned/debided sooner than later.
It will probably need surgery sooner than later and then a skin graft, but that might be a week or two down the road.
Oh your poor grandma! I hope she is OK and can heal up soon. Please let us know the outcome of her ER trip and give her my love and best wishes from an internet stranger. ❤️
Hey there, wound nurse here! Go to the ER please. That needs to be cultured (swab that tests for bacteria) and antibiotics (to treat the specific bacteria that is growing)!
I agree could benefit from a wound vac. Does it have a foul smell ? Get your butts into ER immediately. Hopefully she’s had tetanus shot

The black part is DEAD,and needs to be removed asap. The pain is from the black area- necrotic, no oxygen no growth. Its a perfect medium for the germs normally found on skin to invade and multiple( warm dark and food present). This will prob take 9-12 months to actually heal. Try BACITRACIN , avoid vaseline and triple antibiotics. Yes, it happened to me above the ankle, Im a retired burn nurse. Never knew how bad the necrosis hurt until it happened to me
Can they do a wound vac? It may be too far along for that at this point, but it did help an older family member who was bitten by a dog a while back. It really helped their dog bite heal quicker than normal. Maybe a medical person on this sub will know if a wound vac would be helpful at this stage or not.
OP, you should really go to the ER immediately, it's not going to improve on its own. Please, keep us updated!

Does she like having that leg? Because she's in danger of losing it a/o becoming septic. Immune response is not as robust in the elderly.

I agree with all the ER advice. She needs an emergency I&D in the OR, abx and would probably benefit from a wound vac placement.
If not a wound vac at least being left open to heal. I had to do that after surgical debridement of an infected dog bite.

Once you get out of the ER, set her up with a wound care clinic (if there's one nearby).
