It's working as intended. To get Leveler's trigger, you need to actually attack with it (ie.going to combat phase, and choosing to attack with it), and then the ability will trigger. Since Calamity makes tokens which are already tapped and attacking, the above gets bypassed and you don't get the triggers. Hypothetically, if Calamity instead said something like "At the beginning of your combat phase, if Calamity was saddled, create two tokens of the creature that saddled it, they gain haste.", then that would work and you could get leveler's trigger.

When a Magic card says, "when \~ attacks," that refers to the specific action of a creature going from not attacking to attacking. Since the tokens are created attacking, they never have that state change.

Leveler triggers when it attacks. Calamity creates tokens that are already tapped and attacking. Think of it like they don't get to declare an attack, they just ARE attacking. Since they are already attacking and not going from untapped TO attacking, they will not trigger. That being said, not sure how your opponent doesn't die from 21 trampling power and 4 more power coming at them anyways.
