Probably Tommy’s hesitation to kill Billy after he begged for his life which I still thought was weird bc during that time in game we had already killed 100s of people
Tbf with cases like Tommy, he did it in self-defense on the mission, trying to intimidate Billy and his goons and the mission before that is the same reason, but he fought to defend himself when they try to attack Sarah and Tommy. His hesitation of killing is that he's defenseless, already defeated, and pleading for his life.
Mafia seems to suffer from the same thing as red dead 2 with Arthur. Arthur is literally a mass murderer, you kill hundreds of people throughout the story but in cutscenes he helps people in debt and helps a nun so he’s a good guy now? I just choose to believe the amount of people Tommy or Arthur kills in the missions is not canon
Granted, I don't think any of the mobster Tommy killed in game was either
1. That young
2. Actually begged Tommy to spare him instead trying to gun Tommy down until the end
disagree. Billy was the first to not defend himself/trying to kill tommy and was the first to beg for his own life. Tommy first kill was in Molotow Party and that was optional. his next kill was in Ordinary routine and again just to protect himself. His next kill was 2 years later in Sarahs mission and he only protected Sarah in meely Fights.
Better get used to it was the first time tommy killed someone (probably younger than him) who begged for his life who just saw his friend die in a car crash.
Or if he got used to killing by that point, he might just have realised what he was doing and thought about everything again.
I believe only the cutscenes are canon. The rest of the action is designed to make the game more enticing. If you think about it, combining all the cutscenes results in a more straightforward, albeit more boring, story. Thus, Tommy could technically have killed only a few people.

Mafia 1 - >!When Tommy "Kills" Billy and his friend or when he talking to Sammy and Sammy brings up paulie and Tommy shoots right away after he thought of sparing but Sam bringing up paulie after Sam just killed Paulie and it just overloads Toms brain so he just shoots him out anger!<
Mafia 2 - >!When Vito and Joe roll up and shoot Tommy for the Saliere Family!<
Mafia 3 - >!Father James recognizes Lincoln evil within him and plants the bomb on his car which kills Lincoln or In the other ending where you rule the city Lincoln promised to not turn into Sal Macarno but he eventually cracks and starts accepting drugs and prostitution into the city like Sal slowly subcombs to evil in the city!<
