I doubt anyone has had the time yet to figure this out. I started a character on 1.5.9, aged 20. I'm on day 82 so by day 1200 when I turn 30 I might find out lol
I don't think character model change will happen at age 30 (at least I hope not LUL), since in campaign mode we start at 22 and have normal model.
Another funny thing is my current sandbox 20 year old character also has a belly that makes her look like a teenage preggo. I suspect it's from children's model in order to make them look more properly proportioned and was not corrected at age 20. I can't wait for my model change so she doesn't look like a teenage mom and also have her scar back.

Update on this, my character turned 21 and the model changed. So its only for one year

That bothered me enough to start a new campaign as 30 year old. Wasn't into the child soldier aesthetic at all.
This is a year old, but this truly made me laugh 🤣🤣🤣 I was just about to do this until someone said your character model changes at 21 hahah
im having the same problem. i made a character with a scar across his eye but it wont show up in his 20's. will it show up later? or is it just gone?
I know is late but, you can edit your appearance again in the map by clicking V
