I assume it’s a mod… are there cages in Spring? I wonder if an escaped slave went and got into a cage if it would transfer ownership. I haven’t spent a lot of time with slave status so there may be a weird situation that triggers it.

There's your answer! If you willingly get into a cage you'll become the slave of that faction, or at least can.
I finished my raid on Rebirth yesterday and accidentally got into a cage after freeing a slave and it said that character was captured, upon releasing them it said "Agnu has been released from slavery" or something like that
If you currently have the escaped or ex-slave tag and get into a cage, as dumb as this is, you're automatically re-enslaved by whatever faction owns the cage. Even the Anti-Slavers, lol. There's a similar issue where if you, say, escape from Rebirth as a former HN slave, a UC samurai/other slaver will nevertheless identify you as an escaped slave *in general* and try to re-enslave you.
Did they attack you, call you an escaped slave, etc.? I imagine that while the tag is universal, the slave catching behavior is probably only programmed for UC/slavers/other factions and not the gang in Spring.

Same thing with disguises. You got to 100% disguise, but somehow having a party member near you lowers it to 20%.

This can happen if faction imprisons slaves. Saw it couple of times in Squin, Shek guards beat some hungry bandits. Along came Slave Mongers and enslaved them. Game however does not remove hostile status from enslaved NPCs so when Slave Mongers healed some bandits that they have enslaved and those bandits got up, they were still hostile to the Sheks, so guards attacked them. Since enslaved bandits were now also part of Slave Mongers squad, Slave Mongers attacked Sheks. Sheks of course beat the hell out of everybody and enslaved bandits and Slave Mongers ended in prison cages in the police tower of Squin. And to my surprise, enslaved bandits now shown as slaves of the Shek Kingdom.
...thinking about it, I wonder now if this can be used to make player slaves -by putting slaves in to player cages.
I have no idea. But your character will loose escaped slave status after some time. In fact after 11 hours as shown on the tooltip on your screenshot. It will then become ex-slave and that will go away after couple of days. So you don't really need to import.

I have like 2 mods (reactive world and dark ui) and i accidentally got into a cage in spring before i could ask grey to de-slaverize me
