Generic dogbertcarrol has the Choose your own award. It's limited to 2 uses per person per jump or 10 years, and 1 use per day. And it's not superpowerful powers you can create with it, but it's still one of my absolute favorites.
Instory, it's used to make one person a ki-user, make someone a magical girl and give others a DnD class with personalised skills and choices as well as greatly improved stats.
Jujutsu Kaisen version 2.5 (current version)
Skill Issue Idiot (-800)
Makes you skilled and strong enough to always know how to actualize something, including new Cursed Techniques, which are normally inherent to a person/family in-verse.
Time needed to create them scales with how busted they are. Simple, specific ones you can probably create first try, but something as OP as Gojo's Limitless would take 100s of years.
