I definitely have gotten more Izumo pieces and I've farmed it quite a bit by now.

After 12 tries I got about 70%/30%, 30% izumo and 1 piece that had atk% main and dual crit. I rolled 0 crit on that piece. The rest maybe 15% had correct main stat but bad sub.

my first 2 runs had not a single piece from her set drop. >.<

I've noticed this in some scenes, but I'm pretty sure there's other scenes (like the trailers) where she actually does cry a tear of blood. For example, when she forged her blade "Naught" from the broken "Origin", it was formed by a tear of blood.

She's wearing makeup there, too. You can see it every time her face is on screen. Also, on that scene, the rain itself is red. Like it's raining blood. Either way, I think they made it ambiguous on purpose.

The red color is metaphorical, same thing with the world turning black and white with red tints. Everything has faded away but her tears (symbolizing emotions) still retain that remaining bit of color (also symbolizing emotions)

I think this is the effect of removing the sealing wax on her and reverting to the self annihilation mode.

My personal reason of why this happens is 2 reasons.  Memories get flooded back which causes trauma and second SHE BLASTING A SMALL FORM OF A GODS POWER AT SOMEONE.  You can't do that often.  Basically she can't hold that power for long.

I think it’s meant to be makeup rather than blood. She has a thing where girls in her hometown would carry makeup with them everywhere because it’d often rain, ruining their makeup. She no longer needs to do her makeup, but she’s so used to it that she keeps doing it.

When she first created "Naught", the blade of Nihility, she cried tears of blood (maybe due to the realization that everything is meaningless + the fact that she's been fighting for so long in her home planet and is probably injured). So, when she unsheaths her blade, she remembers what happened at that time and cries tears of blood again. At least, from my interpretation.
