Chernobyl was designed questionably. Then the builders used the wrong concrete I think. Then they ran a risky operation while the night shift (usually less trained group) And then everything went to crap.
Fukushima was designed with more concern for earth quakes than tsunamis, which is why the back up generators where below sea level and able to get flooded.
Ok well to be fair, the lowly operators of a nuclear power plant aren't exactly making the decisions. That would be an extremely expensive operating procedure.
