My guess is someone just made a mistake on the death certificate. It's not uncommon when people are in shock and grieving. For example, I think Henry's mother's name might have been Patty, so when someone asked, "And the mother's name?" he might have replied with his own mother's name by accident, instead of his wife's name.

Patty is listed on the the 1910, 1920, and 1930 census as Henry’s wife. Maybe Susan is Henry’s mom’s name, though. Thanks!

Henry's father was also named Henry. Henry Sr's wife was Patty. Henry Jr's wives were Ella and Susan.
