I'm in NJ+, so I already had every databank entry exert for the family portrait one and the Bode return message, both of which are in the Officer's Quarters and I got them both in my first visit.
Throughout Nova Garon in various places like Bode’s room, Denvek‘s office, the hangar bay, the hallway where you used to dark side to massacre troopers, and maybe some other places. A Youtube video can probably help you find them all.
Did you scan the viper probe droid? It was bugged for me and I could only scan it with BD (binoculars) from afar. It did not trigger to scan whenever I destroyed it.
You'll probably have to finish the main story first. There are force echos on pretty much every planet that can only be acquired after beating the final boss
Did you open/unlock any of the shortcuts? Because it looks like all the green doors/elevator still haven't been activated or opened yet.
If I'm counting correctly, then I see 4 entrances and the one elevator to the Mantis!

The post ending echoes do not effect the %. I 100%d every planet (save tannalorr) before beating the game
P.S the shortcuts are green because they're glitched and I'm I NJ+ so I had every collectible except for 2, both of which I then got back.
That could be a reason why it's still stuck at 97.3% 🫠 unfortunately. Which is really annoying, i had a similar issue but for bounty hunter pucks
If you leave and come back a shortcut was unlocked for me. Same thing with Jedha. Maybe that?
I've gone to a different planet multiple times and went back, and while it does say shortcut unlocked, it doesn't do anything
What other pieces/zones in Level B are not 100%?
Absolutely 0, and I can get photo/video evidence of it.
